I review mistakes made in the Freeh Report and choose one mistake. I need help with the following.

In Fall 2011, Penn State’s Board of Trustees hired Louis Freeh, former FBI director, to carry out an internal investigation of how the University handed the sex abuse allegations against Jerry Sandusky and how the University handled the allegations. “The Freeh Report” (the Report) is the result of that investigation.

I am in a group and my part is to discuss the following from the mistake chosen (attached) by the group.

Write the mistake management ideas and ways to mitigate consequences (cite sources) in the mistake chosen for discussion. I basically need to know what could have been done to prevent this mistake.
