MSN 6011 Unit 2 Discussion Ethics in EBP and Levels of Evidence

Note: Remember that there will only be one discussion per
unit for most of this course, because they have been designed to give you opportunities
to conduct additional research. This will require more of your time than
discussions you have had in previous courses. Please plan accordingly.

This discussion will be based on the case study, Georgia
Health Information Technology Regional Extension Center – Helping Eligible
Providers Reach Meaningful Use, from this unit’s readings.

For this discussion, imagine you are practicing in a rural
setting or in an organization that is similar to the situation described in the
case study. You have been asked by the organization to briefly explain a
potential course of action based on the case study, including ethical
considerations related to that course of action. You have also been asked to
present evidence that would support your course of action or help inform its
development or implementation.

The response for this discussion will contain two parts. You
will need to address the bullet points under each part to successfully complete
this discussion.

Part 1: Ethical Considerations and Implications for Planning
and EBP

Explain a potential course of action, based on the case
study, that could help address the disparity in the availability of quality
health care in rural areas.

Describe the ethical considerations, issues, or dilemmas
that the case study presents health care professionals, and that are relevant
to your potential course of action.

Explain how these ethical considerations influenced the
types of evidence and resources that you searched for and will present in Part
2.

Part 2: Relevant Resources and Determining Levels of
Evidence

Identify a minimum of three credible resources that are
relevant to the case study, your potential course of action, and any ethical
considerations.

The Capella University Library is an excellent place to
start your search for these resources.

For each resource, address the following (each point only
needs to be a sentence or two):

Identify the levels of evidence in the resource.

Refer to the Evidence-Based Practice in Nursing & Health
Sciences: Review Levels of Evidence library guide, linked in the Resources.

Explain its relevance to the case study and your potential
course of action.

Explain how the resource helps you address the ethical
considerations you described in Part 1.

Explain why the source is credible.

Response Guidelines

Compare the ethical considerations you described to those
described by your peer. For considerations that were different between your
posts, comment on how the difference in ethical considerations may have steered
your initial post in a direction that was different from that of your peer.

Briefly explain your current practice setting (or the
practice setting your would like to be working in). How would your peer’s
potential course of action likely play out in your practice setting? What
conclusions can you draw from this about applying evidence-based solutions from
one setting to a different setting?

I agree that medical practice problems are diagnostic and curative in nature, whereas nursing practice problems are concerned with nursing diagnoses, nursing assessments, or nursing processes/situations. They are very different, but they can use the same PICOT format and get very different results. Interesting. One of the articles I read was more focused on the medical PICOT, but it contained a statement that is applicable to nursing practice. According to PICOT, “a lack of [clinician/nurse] representation in research has the potential to limit growth and new developments for the profession” (Riva et al., 2012). It emphasizes the importance of research and involvement in the profession. I completely agree, but before embarking on this journey to obtain my bachelor’s degree, I had no idea what research was or how it related to nursing. I’m curious where in their education physicians are introduced to the world of research. Clearly, very few people in the medical profession are actively engaged in research. I believe it is far more important than I ever imagined in order to keep the nursing profession current and on top of an ever-changing body of knowledge.
Evidence-based practice is a decision-making approach that uses the best available evidence to inform clinical decisions. As a result, healthcare researchers make decisions based on a combination of research evidence, clinical expertise, and patient preferences and values (Helbig, 2018). There is mounting evidence to support EBP in healthcare. EBP has been shown in numerous studies to improve patient outcomes, lower costs, and increase provider satisfaction.
Nursing practice appears to have similar patient goals as medical practice, but both take different approaches to addressing the patient’s problems and improving prognosis. While medical practice focuses on diagnosing the disease or condition affecting the patient or a population, nursing practice focuses on identifying and addressing all health problems that contribute to the medical diagnosis. As a result, even before or without a medical diagnosis, nurses can manage the patient’s symptoms immediately in order to save the patient’s life and improve the prognosis (Kim et al., 2020). However, in order to make an accurate diagnosis, the physician should perform various tests to support the subjective and objective data.

Nurses use the nursing process to ensure effective and long-term solutions to the patient’s illness and health outcomes, including assessment, diagnosis, planning, implementation, and evaluation. Because nursing practice focuses on holistic care and extensive addressing of the patient’s concerns, the PICOT question should be based on a nursing practice problem. As a result, while medical practice focuses on the patient’s pharmacological and possibly non-pharmacological treatment, nursing practice investigates the patient’s various healthcare concerns (Helbig, 2018). As a result, nursing practice will address the issues to ensure that the patient leads a healthier lifestyle, lives in a conducive environment, and receives attention for health issues other than the primary complaints. Furthermore, nursing practice extends beyond curative services to promote health in individuals and communities. In light of these facts, a PICOT must be nursing-based in order to achieve the desired results.

Late Policy

The university’s policy on late assignments is 10% penalty PER DAY LATE. This also applies to late DQ replies.

Please communicate with me if you anticipate having to submit an assignment late. I am happy to be flexible, with advance notice. We may be able to work out an extension based on extenuating circumstances.

If you do not communicate with me before submitting an assignment late, the GCU late policy will be in effect.

I do not accept assignments that are two or more weeks late unless we have worked out an extension.

As per policy, no assignments are accepted after the last day of class. Any assignment submitted after midnight on the last day of class will not be accepted for grading.
